Description

Offers virtual educational workshops for individuals with chronic health conditions. Information and Counseling More information: - Focuses on managing health to improve quality of life and reduce health complications. - Serves as a hub for evidence-based programs. - Serves as the training and learning collaborative for Chronic Disease Self-Management Education, Stepping On and PEARLS programs. - Contracts with multiple health systems across the state of MD as well as Area Agencies on Aging and organizations who serve older adults and adults with disabilities to deliver programs. - Provides a continuum of care which includes lower health care costs, improved health outcomes and improved quality of life for older adults.

Serves as the area agency on aging for Wicomico, Somerset, and Dorchester Counties. Single point of entry linking seniors and caregivers with local services and programs.
